How to Report a Claim
To report a property or casualty claim, click the Automobile, Property, or General Liability button below to access the appropriate forms. Email the completed loss notice to claims@naico.com or fax it to 405-258-1329.
To report a claim by phone, call our toll-free number 800-332-2210 and tell the operator you need to report a new claim. Please have the following information available when calling: Policy number, named insured, type of loss, a detailed description of what occurred, as well as the date, time and location of the occurrence.
When reporting a claim, it is always important to provide as much information as possible. Details of the loss can help expedite the process of filing the claim.
Our regular office hours are from 8:00 a.m. to 5:00 p.m. CST, Monday through Friday. If you need to call and report an EMERGENCY CLAIM after hours, call our toll-free number, and you will be directed to our answering service. An EMERGENCY CLAIM is defined as a claim involving major injuries, lacerations requiring hospitalization, fatality, major property damage, environmental hazard or business interruption. Please note that our answering service will not record the details of the claim. They will ask you to provide them with your contact information, and a representative from NAICO will contact you shortly.

Reporting a Workers' Compensation Claim
To report a workers’ compensation claim, click the appropriate state below and complete the Employers First Report of Injury form. If the state isn’t listed, select the “All Other States” form at the bottom of the list. Read the instructions carefully and provide as much detail as possible. For Lost Time claims, complete the Employee’s Wage Information form below. Keep a copy of the completed forms for your records, send a copy to your insurance agency, and email a copy to wcclaims@naico.com or fax to 405-258-3005.
