How to Report a Claim

The NAICO claims procedures have been designed to eliminate red tape and delays. In order to be as efficient as possible, we must have some information from you. Your claims representative will need as much information as you can provide. No detail will be too small. Your representative will first make sure you have a valid policy in effect and then your policy language will be examined to determine applicable coverages. NAICO will protect your best interest in accordance with your policy provisions.

NAICO prefers all claims be reported by telephone. To report a claim, call NAICO at 1-800-332-2210. You may also file a claim by fax or mail.

Fax: 405-258-3005

Mail: P. O. Box 38
        1010 Manvel Avenue
        Chandler, OK 74834

Before reporting a claim by telephone, have the following information available. You will be asked to explain what happened. Try to be as accurate as possible about dates, times and persons involved. Be ready to supply any witnesses’ names and telephone numbers. NAICO may require your assistance in the handling of the claim by asking for more information (statement of the facts, receipts, repair estimates, medical reports, etc.) Every effort will be made to process your claim in the shortest time possible.

To help expedite your claim, have these items available:

  • Policy Number
  • Named Insured
  • Type of Claim and Detailed Description of Claim
Dates, times, who was involved, locations, witness names and contact information, detailed description of accident (no detail is to small)
  • Claim Data
Receipts, repair bills, medical bills, repair estimates, other relevant reports.

If you report a claim by fax or mail, use a sample of a first report of a claim form. This form can be completed and faxed or mailed at the above contact information; however, we prefer that your claims be reported by phone so we can immediately begin providing the claim services that your desire and deserve.
